Five Easy Steps to Access Your Survey Kit Instructions
1. Login using your CAP user ID.
2. Select Proficiency Testing/Quality Management.
3. Click Result Forms to be directed to the Result Form Data Entry and Receipt Verification page.
4. Choose an available kit from your laboratory list or enter the kit number in the Advanced Filter options. Click View in the Details column.
5. Click Kit Instructions under Related Links to begin your Survey.
