Residents Forum Delegates who register and attend 2014 RF meetings are eligible for reimbursement. Attendance at one RF meeting in the calendar year enables the delegate up to $100 in reimbursement. Attendance at two RF meetings in the same calendar year enables the delegate up to a total of $300 in reimbursement. Receipts must be provided for the amount for which you are eligible. To claim your reimbursement, follow these instructions:
- Register online for the meeting so we know you are coming.
- Sign in at the RF registration desk the morning of the meeting so we know you were there.
- Complete the member reimbursement form (Excel, 150 KB) and send it in along with the appropriate receipt(s) for the amount you are claiming. Send all forms and appropriate receipts to Jan Glas via email or fax: 847-832-8499.
