Objective: To monitor the type and number of wristband errors and identify which policies and procedures have an impact on wristband error rates.

95-08 Autologous Blood Transfusion Practices
Objective: 1) To measure transfusion rates for selected elective surgical procedures for which autologous blood collection is sometimes requested; 2) to determine patterns of allogeneic blood transfusion, and patterns of autologous blood donation and transfusion; 3) to calculate how well autologous donation serves to minimize patients’ exposure to allogeneic blood; 4) to compare autologous and allogeneic blood transfusion rates; and 5) to assess the risk of mishaps involving the transfusion of autologous blood.

95-09 Autopsy Result Utilization
Objective: To determine the methods by which preliminary autopsy results are communicated to the appropriate persons and how communication is established with the decedent’s family. In addition, to determine the frequency that autopsy findings are utilized in intra- and interdepartmental activities and the destinations for final autopsy reports sent by the pathology department.
