Examine how well your laboratory prevents identification errors by detecting them before results are released to caregivers. Participants in this study will compare their identification error rates and the percentage of errors detected before results are released to other institutions. This study will also seek to identify institutional processes and factors more likely to be found in laboratories that detect a high proportion of identification errors before results are released. |

Investigate the frequency of urine culture contamination in outpatient specimens collected or processed by your facility, how this rate compares with that of other institutions, and what institutional processes or factors contribute to a lower contamination rate.
