The distinction of low-grade forms of intraductal neoplasia (ductal carcinoma in situ, atypical ductal hyperplasia, flat epithelial atypia) from benign epithelial proliferations represents a recurring diagnostic problem in daily practice. This Video Microscopy Tutorial will explore issues related to the assessment of ductal epithelial atypia in the breast. The session will present criteria for low-grade cytological and architectural atypia and contrast them with the morphologic features of usual ductal hyperplasia. Faculty will apply these criteria to the evaluation of different intraductal proliferations, including flat, micropapillary, and solid lesions. The course will review specific problems in differential diagnosis, such as distinguishing flat epithelial atypia from non-atypical columnar cell changes and solid papillary carcinoma in situ from usual ductal hyperplasia. The course will also include discussion of utility and limitations of ancillary immunohistochemical studies. You will gain competence with practical morphologic criteria that will aid in the evaluation of these problematic breast lesions.
