AP108  Biopsy Diagnosis of Colitis: Challenges and Solutions

Physicians routinely perform colonoscopic procedures as they manage of patients complaining of diarrhea, hematochezia, and chronic abdominal pain. As a result, a major part of routine pathology practice includes the evaluation of colonic biopsies to rule out various forms of colitis. A significant overlap of histologic features between various entities often makes this exercise difficult. For example, differentiating early inflammatory bowel disease (IBD) from infectious colitis can be particularly challenging. Differentiating between drug-induced colitis and ischemic colitis and diagnosing long-standing IBD with dysplasia are also difficult dilemmas. In this course, discussion will highlight the common errors in diagnosis and offer a systematic approach to the proper identification of colitis in mucosal biopsy. In addition to an in-depth review of the various forms of colitis, faculty will underscore the importance of correlating the endoscopic appearance and clinical information with the microscopic findings. Using a case-based approach, topics will include infectious colitis, iatrogenic colitis, ischemic colitis, lymphocytic and collagenous colitis, IBD and its mimickers, and IBD-associated dysplasia and its management implications.
