Diffuse malignant mesothelioma is a rare malignant neoplasm with a dismal prognosis, the diagnosis of which carries serious medical and legal consequences. Its rarity, variable histology, and similarity to other lesions—both neoplastic and nonneoplastic—as well as to small tissue samples, are challenges to its accurate diagnosis. Pathologists must be able to accurately diagnose diffuse malignant mesothelioma and distinguish it from its neoplastic and nonneoplastic mimickers. Learn the variable histologic features of diffuse malignant mesothelioma and review the diagnostic utility of immunostains; discuss diagnostic criteria helpful in distinguishing its neoplastic and nonneoplastic mimickers and the reactive process from neoplastic mesothelial proliferations; examine the challenges in its diagnosis outside the pleura; and stay on top of the latest issues regarding causation and asbestos exposure.
