Ordering Tips
- If you are a current CAP customer, enter your CAP number in the upper right. If you are not a current CAP customer, a CAP number will be assigned when your order is received at the CAP.
- Include your CLIA number, provided by CMS as a part of your CLIA certification, if available.
- It is important that you provide a complete address, telephone number, fax number, e-mail address, and contact name on your order form. (PO Box numbers are not acceptable.)
- Select the product modules you need by filling in the bubble to the left of the specific item. (If you would like to purchase multiple quantities of any of the product modules, please call the CAP Customer Contact Center at 800-323-4040 option 1 #.)
- To avoid delay in the processing of your order, one of the following methods of payment is required:
- Check payable to the College of American Pathologists
- Credit card number, expiration date, and authorized signature
- Purchase order number
- Letter of authorization – if your institution does not use a PO system, a signed letter of authorization on institutional letterhead will be accepted.
Order terms are 30 days from the date of the invoice.
Tax Information
If your institution is subject to sales tax, the appropriate tax for your locale will be added to your invoice. If you are not subject to sales tax, please enclose your tax-exempt certificate with your order form. The CAP federal tax identification number is 36-2118323.
Additions, Cancellations, and Refunds
The CAP will make every attempt to accommodate orders received. Due to vendor commitments, cancellations must be received six weeks prior to the ship date to receive credit for canceled products. Credits issued on your account should be applied to open invoices. If there are no open invoices on the account, the CAP will issue a refund.
Product Delivery
- All shipments are made FOB Origin.
- The CAP is not liable for loss or damage caused by force majeure.
