The Laboratory Accreditation Program is pleased to announce changes to streamline and simplify the process for completing the CAP forms for individualized quality control plans (IQCP). Effective immediately, the CAP has discontinued use of the IQCP Summary form and has updated the List of IQCPs form to include new fields to allow one form to be completed for inspection preparation.

The CAP recently released a new Fast Focus on Compliance (FFOC) mini-training module to update histocompatibility laboratory personnel and inspectors on changes to the 2016 edition of the Histocompatibility checklist.

You previously received a notification from the College of American Pathologists' (CAP) Laboratory Accreditation Program (LAP) regarding the discontinuation of required proficiency testing (PT) for waived whole blood glucose on glucose meters effective in 2017.

As a service to CAP-accredited laboratories, the CAP is providing information and resources regarding a change that will be made in the 2016 Checklist edition, scheduled to be released August 17, and to the Laboratory Personnel Evaluation Roster form.

This notice is to alert you that the College of American Pathologists' (CAP) Laboratory Accreditation Program (LAP) will no longer require enrollment and participation in proficiency testing (PT) for waived whole blood glucose on glucose meters (ie, WB2/WBG) and waived whole blood Protime/INR (ie, WP10) beginning with the 2017 PT program year. Laboratories will be required to perform alternative performance assessment for these analytes.

As a service to CAP-accredited laboratories, the CAP is providing the following information to help maintain continuous compliance. This is in response to changes in the Centers for Medicare & Medicaid Services interpretive guidelines, effective January 1, 2016, that will remove the Equivalent Quality Control option and replace it with the Individualized Quality Control Plan option.

Effective immediately, the histocompatibility section director may define the level and nature of the supervision required for personnel gaining the required one year of training and/or experience in histocompatibility testing.

As a service to CAP-accredited laboratories the CAP offers compliance alerts to help your laboratory maintain continuous compliance. A recent article published in the July 2015 edition of CAP TODAY, "IQCP worries? Help with what ends and begins," included information that the state of Kentucky is not allowing the use of the Individualized Quality Control Plan option to meet quality control requirements.
