- [Norman Moore, PhD](https://www.cap.org/people/norman-moore-phd/): Dr. Moore received his bachelor’s degree in biology and philosophy from Dartmouth College and his PhD in microbiology from the University of New Hampshire. Among other assays, he developed the first ever rapid tests for Legionella and S. pneumoniae, both of which are now recommended by the Infectious Disease Society of America. After being an R&D director for many years, he took the role of Director of Medical Affairs where he works with the medical community. He has served on multiple NIAID grant committees, the CDC guideline group for rapid influenza testing, the College of American Pathology’s and Clinical Laboratory Standards point-of care committees, and CLSI’s infectious disease guideline writing committee. He currently has eight US and 39 non-US patents and numerous publications and presentations. He received the Volwiler Fellowship Award for lifetime achievement in science in 2019.

After completing combined residencies in anatomic and clinical pathology at the Cleveland Clinic in 1987, Dr. Myles joined the faculty of the Medical College of Ohio before being recruited back to the Cleveland Clinic in 1993. Dr. Myles has served in a variety of roles at Cleveland Clinic, including AP education coordinator, director of the autopsy service, director of histology and the surgical pathology desk, and most recently as the Quality Institute Officer for the Robert J. Tomsich Pathology and Laboratory Medicine Institute at Cleveland Clinic. Dr. Myles has practiced both anatomic and clinical pathology and currently serves as a subspecialty pathologist in genitourinary pathology.

He has faculty medical appointments at Yale, University of Connecticut, and Quinnipiac University. He did his pathology training at Yale and Memorial Sloan-Kettering Cancer Center and his forensic pathology fellowship at the New York City Office of Chief Medical Examiner. Prior to Connecticut, he was the Deputy Chief Medical Examiner for Bronx County. Seven are Merit-based Incentive Payment System (MIPS) clinical quality measures (CQMs). The remaining six are pathology measures developed by the CAP and approved by the CMS as qualified clinical data registry (QCDR) measures—exclusively available in the Pathologists Quality Registry. To better understand how to review and interpret measure specifications, the CMS has developed Quality Payment Program Measure Specification and Measure Flow Guide for MIPS Clinical Quality Measures.

Pathologists will have the new CPT codes for pathology clinical consultation services in 2022. The CAP worked with the American Medical Association’s (AMA) CPT Editorial Panel to establish pathology clinical consultation codes and develop new payment rates through the CAP’s role in the AMA/Specialty Society Relative Value Scale Update Committee (RUC). - [Digital Pathology Codes](https://www.cap.org/advocacy/payment-and-reimbursement-policy/digital-pathology-codes/): The College of American Pathologists (CAP) worked with the American Medical Association (AMA) CPT Editorial Panel to establish 30 new digital pathology add-on codes (Table 1: CPT 2024) for 2024. Digitization of glass microscope slides enables remote examination by the pathologist and/or in conjunction with the use of artificial intelligence (AI) algorithms. Category III add-on codes 0751T-0763T, and new codes 0827T-0856T are intended to capture and report additional clinical staff work and service requirements associated with digitizing glass microscope slides for primary diagnosis. As a result of CAP advocacy, the new codes will help pathologists, pathology practices, and laboratories providing digital pathology digitization procedures appropriately report these services. The Digital Pathology CPT instructions were also revised clarifying that each Category III add-on code is reported as a one-to-one unit of service for each primary pathology service code (CPT Instruction and Guidelines). The new Digital Pathology codes will be effective January 1, 2024.

## Challenges
- [Omentum](https://www.cap.org/pathology-case-challenge/omentum/): A 14-year-old girl presents with a 9 cm complex cystic mass in the right ovary with a 2.9 cm solid component. During the resection procedure, numerous 0.1 to 0.2 cm well-circumscribed white plaques are noted and biopsied along the omentum and peritoneal surfaces. The ovarian mass is a grade 3 immature teratoma (not shown). Exhaustive sampling of the peritoneal nodules showed a consistently homogenous appearance, and the cells stain for S100 but not SMA.  
- [Anterior mediastinum](https://www.cap.org/pathology-case-challenge/anterior-mediastinum/): A 65-year-old woman undergoes computerized tomography (CT), which reveals a mass in the anterior mediastinum. Gross examination of the resected mass shows a well-encapsulated, tan-white, lobulated 6 cm mass with cystic changes. No gross hemorrhage or necrosis are identified. Histologically, the tumor consists of a nodular proliferation of epithelioid cells sharply separated by an abundant lymphoid stroma. These epithelioid cells are positive for cytokeratin AE1/AE3, p63, and PAX8 and negative for CK20, synaptophysin, chromogranin, CD20, CD5, and CD117. The intervening lymphoid stroma is largely positive for CD20+ B-cells intermixed with CD3+ mature T-cells. CD3+, TdT+ immature T cells are seen at the interface of nests with the lymphoid stroma.

Order Additions and Cancellations The CAP will make every effort to accommodate orders received after December 1. Orders are dependent upon the shipping schedule, specimen availability, and specimen stability. Due to vendor commitments, the CAP must receive cancellations no later than six weeks prior to the published ship date to give credit for canceled programs. Program shipping fee refund requests will not be honored if received within the six-week period prior to the first scheduled shipment date. The CAP does not accept the physical return of PT/EQA programs. Participants are responsible for all costs and expenses related to the return of anatomic pathology slide shipments to the CAP if the reason for such return is late program cancellation, refused shipment, or invalid ship-to address. Products received in unsuitable condition to perform testing for any reason may be rejected and discarded by the customer. The customer will receive, at its option, a full credit of the purchase price or the replacement of the products at no additional charge and with no additional cost of shipping. Prelicensure/Reinstatement Service The CAP provides PT/EQA samples to laboratories that may need to perform prelicensure testing before beginning patient testing and/or reinstatement testing due to PT failures. Contact the CAP for cost and availability. PT/EQA samples with limited stability are not available for prelicensure/reinstatement testing. Reinstatement evaluations will be provided upon submission of results to the CAP. CAP and CLIA Numbers Please have your CAP number available when placing orders. For new CAP customers, a CAP number will be assigned to the laboratory and will appear on you order confirmation. For all customers subject to CLIA: Confirm or provide the laboratory’s CLIA identification number, which is assigned by the Centers for Medicare & Medicaid Services (CMS) and can be obtained from your laboratory director. Contact your CMS Regional Office if you do not have a CLIA identification number. Orders may be submitted pending assignment of your CLIA number. Forward the number to the CAP once assigned. Prices/Estimated Costs for Laboratories Outside the US Prices exclude taxes, duties, brokerage fees, disbursement fees, and other applicable shipping and customs charges.
