CLFS reporting open through July 31

The Centers for Medicare & Medicaid Services (CMS) is accepting submissions in the CLFS module through July 31.

What to do:

  • Register in the CLFS module as a Submitter (to enter data) or Certifier (to review and certify submissions). Allow time for identity verification, which may take several days.
  • Prepare your data in advance using the CMS template and confirm which HCPCS codes are included.
  • Report each private payor rate and volume separately for each test and code.

Important: Failure to submit individual rates may result in your data not being included and could lead to penalties for misrepresentation.
